Welcome to the Office

Thank you for choosing our office for your medical health care needs.  To assist us with your care, please arrive at least 15 minutes early to complete the registration forms.  Also, please bring your insurance card and a photo ID.

We have created a comfortable environment for you and our friendly, professional staff is always available to assist you with any questions or concerns you may have.

We ask that you bring all prescription medication with you to your appointment.  Please request all refills of continuous medicines during your appointment.

If you do not have health insurance, we request payment be made at the time services are rendered.  You may contact our billing department to develop a payment plan that best meets your needs.

Office Policy

Office hours are by appointment.  For an appointment, please call (215) 348-4478.  We will accommodate a same day appointment should you require one.  If you find it necessary to cancel your appointment, kindly give us at least 24-hour notice.

Insurance

Please bring your insurance card and a photo ID to your appointment.  At the time of your visit, you will be asked to pay all copayments, coinsurance and deductibles if applicable.  We accept, cash, personal checks and all major credit cards for your convenience.  We will file insurance claims on your behalf.

Referrals

Many medical insurance companies require written referrals from your primary care provider before they will cover services that cannot be provided in our office.  An appointment with your provider may first be required in order to evaluate whether a referral is needed. 

Except for medical emergencies, the insurers usually require the referral be done before the service is carried out.  They may not approve referrals written after the service was obtained.

All referrals in our office are transmitted electronically therefore eliminating a need to make a separate trip to pick up referral forms.

Contact our office to request a referral.  Messages may be also be left in our referral voice mailbox. Please allow 24 hours for processing refill requests. Please keep in mind an appointment may be necessary to determine need for a referral.

Handicapped Accessibility

The office has four ramps available from the parking lot, wheelchair-accessible restrooms, wide hallways, and a ramped entrance.  A complimentary wheelchair and walker is available for your use.

Prescription Refills

Contact our office to request a prescription refill.  Messages may also be left in our prescription renewals voice mailbox.  If the medication is able to be refilled, we will submit a prescription to the pharmacy of your choice.  Please allow 24 hours for processing refill requests.

Please keep in mind an appointment may be necessary to determine need for a refill.

Medical Records Transfer

In order for us to transfer your medical records to another office, we need to have a Medical Records Release form signed and dated by you.  This allows you to specify to whom we are able to release your medical information and for us to protect your medical information under the HIPAA regulations.  Please allow ten (10) business days to process your request.  There may be a charge for copying medical records.  Please contact us for additional information.

Quality Improvement Initiatives

We  routinely submit surveys to patients.  We take seriously any way we can improve patient satisfaction and will continue to monitor for improvements.

We submitted a survey to patients and found the ease of getting through to the office on the phone was 95% and the timeliness in our returning phone calls was 96%.We are also randomly documenting the times of each call and the time of the call back to monitor timely call backs to patients. Urgent and After-Hours Care

We provide many services that might help our patients avoid long waits at the emergency room. We have space available every day for urgent problems.  Please call our office first prior to going to the emergency room or urgent care facility.

For truly urgent problems that arise when the office is closed and cannot wait until the office reopens, a provider is on call seven days a week, 24 hours a day and can be reached by calling the regular office phone number.  Please have your pharmacy’s telephone number available in case you require medication.
